If you defined a minimum time between showing the captcha and receiving
the result you could prevent most of that.  

still don't like captchas , unless they use kittens

I use a long list of questions ( usually humorous and specific to the
site contents) of which one is chosen at random when users post or
create a new account. This keeps spam at bay on my phpbb forum whilst
not breaking usability.

This is done with the 'Textual Confirmation' phpBB mod.
