I'm about to start reselling a hosted Zimbra solution that ticks all  
of these boxes - because I have the same requirements - drop me a  
line off list if you're interested. Anyone else too. I'll be keeping  
it as cheap as I can because my primary motivation is to provide a  
reliable service (for clients/partners/myself) rather than because I  
see it as a big earner.

Also supports Outlook integration, Apple iSync and Blackberry style  
mobile push.
